Ram and Janani fall in love as schoolmates in Chennai and, after overcoming the objections of her family, marry young. Their happiness is real, but Ram has begun to struggle with something he cannot name or control, and as his bipolar disorder takes hold he hides it even from his wife, with only his close friend aware of what is happening. The film opens with tragedy and moves back in time to show how the couple got there. 3 was the directorial debut of Aishwarya R. Dhanush and the first film score of Anirudh Ravichander, whose song "Why This Kolaveri Di", sung by Dhanush, became an internet sensation across India months before the film's release. Sivakarthikeyan, then a television host, appears as Ram's friend.
